Just downloaded "Flight Card" from the OAV dispatch office :

From the read-me :

"Project “Flight Card” aims to gather as much valuable info can fit
in a single page in order to make your life easier while conducting
a flight.

The included “Flight Card.pdf” is to be printed prior to an IFR flight
in order to write down pre-flight data as well as to be used during
the flight in order to write down in-flight data.

This is version 2 of my "Flight Card" project. Several adjustments
were made with the advice of Evagelos M. Vaos (PhD) and
Nikos Gerodoukos (Captain, B737), who I thank for their
contribution.

This product is freeware. I hope you'll find it as useful as I did.
All recommendations for future improvement are welcome.

"WIND COMPONENT ADJUSTMENT
Distance (nm) : of the T/O runway ?
Length (hh:mm) : of the T/O runway ?
Wind :
Adjustment (nm) : ?
Nautical Air Miles : ?

How to use/compute that kind of info. Could you please provide an example ?

I assume distance refers to the length of the trip in nm (i.e. 2000nm). Same for the length as the time of flight (i.e. 3hr:30mn). Now if you have a headwind component of 50kts (i.e. 50nm/hr) the adjustment for 3.5hrs of flight becomes 3.5x50=175nm. Therefore since it is a headwind your adjusted distance becomes 2000nm+175nm=2175nm. This is just a guess..
